Can you elaborate on what Sus Plus 4 Chord is?

If a Sus2 replaces the third with a second and a Sus4 replaces the third with a fourth

What do you mean by a Sus plus 4 chord?
If the 4 refers to the 4th of the scale... D .... how does D# or Eb fit into it as a 4 ?
